Virtual RN Care Manager – Complex Care
Role Overview
- Salary: $85,000–$90,000
- Location: 100% Remote / Work From Home (EST Schedule; all patients reside in New York)
- Panel Focus: Manage an assigned panel of 80–100 high-risk, post-discharge adult and geriatric patients
What You’ll Do
- Panel & Care Plan Management: Conduct initial post-discharge assessments and build tailored, proactive care plans.
- Proactive Outreach: Perform routine telephonic follow-ups, symptom monitoring, and address social determinants of health (SDOH).
- High-Acuity Re-Engagement: Handle complex clinical escalations, wound care issues, uncontrolled symptoms, and acute deterioration.
- Interdisciplinary Collaboration: Partner with physicians, APPs, Clinical Coordinators, pharmacists, and home health staff.
- Documentation: Utilize Canvas EMR and analytics tools for real-time documentation.
Schedule & Location
- Hours: Monday–Friday, 8:00 AM – 5:00 PM EST
- Meetings: Mandatory daily morning huddles at 9:00 AM plus select weekly team touchpoints
Qualifications
- Licensure: Registered Nurse (RN) with an active New York State license
- Experience: Minimum of 5 years of RN experience in complex medical case management and palliative care
- Preferred: Certified Case Manager (CCM) certification
