About the Role This role supports Avail Health’s work with an Institutional Special Needs Plan (I-SNP) Medicare Advantage health plan. You will be part of a team completing Chronic Disease Assessments (CDAs) while also supporting health risk assessments and care planning for health plan members residing in long-term care facilities, through accurate assessment and clinical documentation. In this role, you will be working from home, collaborating closely with Avail Health mobile nurses who will assist you with the technology setup and real-time coordination to support telehealth visits. This is a highly collaborative, patient-facing role that requires strong communication skills, comfort with technology, and flexibility to work with mobile nurses within your assigned region, as needed for our partnership with the health plan partner. This is a 1099 Contract position. Contract Details Contract Length: 3 months (January through March 2026) with potential to extend an additional 3 months based on program needs and performance. Schedule Commitment: This is a PRN (as-needed) position with shift availability based on organizational needs. Work is offered strictly on an as-needed basis. Shifts are scheduled Tuesday–Friday, between 7:00 a.m. – 6:00 p.m. Telehealth visit times may vary between 8:00 a.m. – 5:00 p.m. Position Type: Remote, work-from-home role covering Ohio, Indiana, West Virginia, and Maryland. Compensation Hourly Rate: $75 per hour (1099 independent contractor) Training Rate: $50 per hour for required training, including mandatory compliance training, Avail Health technology training, and clinical documentation training. Mandatory meetings may be scheduled from time to time to support program alignment or operational needs. These meetings will take place during the Schedule Commitment hours and will be paid at the standard Training Rate. Required Qualifications • Master’s or Doctoral degree in Nursing (MSN or DNP) from an accredited program • Current national certification as a Nurse Practitioner (FNP or Adult-Gerontology NP) • Active, unrestricted Nurse Practitioner and Registered Nurse licensure in the state(s) of practice • Licenses must remain current and in good standing throughout employment with Avail Health. • Minimum 3+ years of Nurse Practitioner experience working with older adults and/or chronic disease populations. Primary care, geriatrics, or value-based care experience is a plus. Mobile and long-term care setting experience preferred.
