A company is looking for a Case Manager (LVN).
Key Responsibilities
• Assess member needs across various health domains and coordinate care plans
• Connect clients to necessary social services and advocate on their behalf
• Maintain documentation and report on program compliance and client progress
Required Qualifications
• Active California Licensed Vocational Nurse (LVN) license
• 2+ years of experience in care management or related fields
• Working knowledge of government and community health resources
• Willingness to work Monday-Friday with potential evening and weekend hours
• Bilingual (English/Spanish) preferred
