A company is looking for an LVN Case Manager.
Key Responsibilities
• Assess member needs across various health domains and coordinate care plans
• Connect clients to necessary social services and advocate on their behalf
• Maintain patient records and complete documentation within established timeframes
Required Qualifications
• Active California Licensed Vocational Nurse (LVN) license
• 2+ years of experience as a care manager or related role
• Knowledge of government and community resources related to social determinants of health
• Willingness to work Monday-Friday with possible evenings and weekends
• Bilingual (English/Spanish) preferred
