Primary Responsibilities:
The LVN provides direct nursing care to an assigned group of patients, under the appropriate supervision of a Provider, Registered Nurse, by applying clinical nursing knowledge, skills, and ensuring the safety and comfort of patients and families according to legal, organizational and professional standards
The LVN prioritizes and makes judgments concerning basic needs of multiple patients to organize care
Assists with outreach initiatives and goals set forth by the Primary Care at Home leadership
Ongoing liaison between patient, multispecialty care departments and approved Facilities

Required Qualifications:
Graduate of an accredited LVN Program
Current LVN License
License Vocational Nurse in the State of Texas
Current BCLS
Ability to travel throughout the greater Houston area to facilities and patient homes
Driver's license and access to reliable transportation and current insurance coverage
Preferred Qualifications:
1+ years of Home Health experience
Care Management/ Disease Management
EPIC experience
Windows based computer skills
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $20.38 to $36.44 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
