Job Summary
We are seeking a dynamic and compassionate Nurse Practitioner or Physician Assistant to join our multidisciplinary healthcare team in this fully remote position. In this pivotal role, you will provide high-quality, patient-centered care for multiple subacute rehab and skilled nursing facilities. Your expertise will support comprehensive assessments, diagnosis, treatment planning, and ongoing management of patients.
This position is for the day shift 7AM - 7PM (3 shifts x 12 hours - Friday, Saturday and Sunday).

Responsibilities:
During hours of work, there will be times answering the call line and when not answering the call line there will be other items to complete.
• Cover our on-call provider line, which currently covers multiple subacute rehab and skilled nursing facilities in across multiple states.
• Review labs, radiology reports, medications, consults, pharmacy recommendations.
• Prescribe medications including narcotics and controlled substances when needed.
• Prescribe and order a variety of IV medications, IV fluids.
• Order diagnostic testing when needed (labs, x-rays, fluids, UA etc.).
• Send to ED or refer to specialists when needed.
• Other remote work as delegated by Director of Remote Provider Services
Requirements
• Valid Massachusetts state licensure as a Nurse Practitioner or Physician Assistant with current certification (e.g., NCCPA certification for PAs).
• Current/Valid Massachusetts DEA license
• Proven experience in subacute rehab and skilled nursing facilities
• Strong familiarity with EMR/EHR systems
