Registered Nurse, Virtual Care

Location: 1685 Tech Ave, Mississauga, Ontario, L4W 0A7 Why is this position important? The Registered Nurse - Virtual Care is licensed to work in Canadaand provides remote patient assessments, triage, education and care management via telephone, video conferencing, and electronic health records. As point of contact for patients, you will provide convenient and timely supportive services that range from minor acute care treatments to health coaching for various disease states. You will coordinate care with multidisciplinary teams. You will be responsible for triaging incoming requests from patients, assessing their needs and providing the appropriate clinical recommendations and coaching to support resolution of symptoms and/or improvement in perceived health. Patients will be able to contact a registered nurse via a variety of channels including phone, email, text and videoconference. Working collaboratively with other health care professionals (Nurse Practitioners, Physicians, Pharmacists and Dieticians), you will provide meaningful patient experience, while using your critical thinking skills to develop, implement, and evaluate comprehensive care plans. What you will do: • Complete initial patient assessments, follow-ups and ad hoc support for patients with chronic disease • Provide remote patient care including coaching and education using digital communication tools (including texting, phone calls, and telehealth platforms) • Conduct one-on-one sessions, offering motivational support to help patients establish health goals and reduce high-risk behaviors. • Respond or initiate telephone calls from callers via virtual assessment and triage • Utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate. • Manage chronic diseases and review care plans • Collaborate with other professionals in providing telehealth care by making appropriate referrals to nurse practitioners, dietitians, pharmacists and other services. • Monitor call flow and contribute to achieving service targets and maintaining standards of care. • Engage in service/program development of the virtual care program, including liaising with healthcare providers and patients, and being a champion and advocate for the program. • Communicate operational or resource changes as necessary and escalate operational or technical issues using appropriate reporting and communication mechanisms. • Maintain accurate, timely, and secure documentation in electronic health records.
