Work Arrangement: Fully Remote (Nevada license required) Schedule: Flexible | Part-Time or Full-Time Licensure: Active Nevada LPN or LVN (Compact licenses not eligible) Role Summary PharmD Live seeks a Spanish-speaking LPN/LVN to provide remote virtual care for patients in CCM and RPM programs. The role focuses on bilingual patient education, care coordination, and clinical support. Key Responsibilities • Conduct telehealth visits in Spanish and English • Review patient-reported data and RPM alerts • Document care in compliance with CMS and internal standards • Collaborate with interdisciplinary care teams • Support post-discharge and follow-up coordination Qualifications • Active Nevada LPN or LVN license • Professional fluency in Spanish and English • Minimum 2 years clinical or telehealth experience • Strong documentation, clinical judgment, and communication skills • Experience with CCM or RPM preferred • Ability to work independently in a HIPAA-compliant environment
