Part-Time Nurse Practitioner (NP) – Telehealth AWVs (NY Patients) We are hiring a part-time Nurse Practitioner to conduct Medicare Annual Wellness Visits (AWVs) via telehealth for patients located in New York State. This is a 100% remote role focused exclusively on preventive care (no problem-based visits). New graduates are welcome to apply. Compensation Up to $75/hour, based on experience and skills. Requirements • Active, unrestricted NY NP license • Eligible to participate in federal healthcare programs (not excluded) • Comfortable using EHR systems and telehealth platforms • Strong documentation skills Responsibilities • Conduct Medicare AWVs via telehealth • Complete HRA, medication reconciliation, screening review • Develop personalized prevention plans • Document thoroughly and accurately • Identify care gaps (follow-ups coordinated by operations team) Preferred • Medicare or AWV experience • Multi-state licensure • Telehealth experience Application Questions • How many AWVs have you completed in the last 12 months? • Have you performed telehealth visits? • List all states where you are currently licensed. • Have you ever been excluded from Medicare/Medicaid?
