Functional Medicine NP Location: Farmington, CT (In-Person, Hybrid, or Remote Considered) Structure: Independent Contractor (1099) Schedule: Flexible (Minimum 2 clinical days/week) Holistic Health Code (hhc+) is a well-established functional and integrative medicine clinic with a strong reputation, consistent patient demand, and a highly engaged community. This role is designed for a Nurse Practitioner who wants to practice real functional medicine with autonomy, depth, and the ability to build meaningful patient relationships.
