[Remote] Licensed Vocational Nurse (LVN) Home Health

They are seeking a compassionate, organized, and team-oriented Full-Time Home Health LVN to serve patients throughout San Francisco, collaborating closely with the nursing and administrative teams to ensure coordinated, high-quality care. Responsibilities • Implement the plan of care initiated by the Registered Nurse • Provide accurate and timely documentation consistent with the plan of care • Assess and educate patients and caregivers regarding diagnosis, care plans, and treatments • Participate in coordination of home health services, reporting additional needs (HHA, PT, OT, ST, MSW) to the RN or Clinical Supervisor • Use equipment and supplies effectively and responsibly • Complete online charting in Kinnser using company-provided devices • Follow all company policies and procedures • Perform other duties as assigned Skills • Minimum 1 year of nursing experience in a medical setting • IV certification and experience with wound care • Active California LVN license • Valid California Driver's License • Home health experience recommended but not required Benefits • Medical, Dental, Vision • 401(k) with employer matching • Life insurance • Referral program • Flexible scheduling Company Overview • Health Link Home Health and Hospice provides responsive in-home health care to individuals and families.
