SummaryThis position is eligible for the Education Debt Reduction Program (EDRP), student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.DutiesHelpThe Remote Patient Monitoring- Home Telehealth Care Coordinator (RPM-HTCC) RN utilizes health information and telecommunication to promote therapeutic relationships and shared decision making between the patient and interdisciplinary team across the continuum of care to support patient self- management, prevention, and wellness. The complexity of duties include patient training on state of the art telehealth monitoring equipment, conducting telephone or video enrollments, daily monitoring using the telehealth monitoring system, frequent telephone patient triage and intervention, counseling and education for health prevention wellness, counseling education for chronic, infectious disease, Mental health and weight management , order entry within approved departmental protocols, frequent and close collaboration with the primary care provider and team to achieve desired outcomes and established Veterans Integrated Services Network (VISN) benchmarks. The RPM-HTCC RN Coordinates referrals of Veterans to other community programs when necessary and assumes healthcare monitoring delivered to Veterans is timely, appropriate, of high quality and cost effective. The RPM-HTCC RN works closely with VA Primary Care and Mental Health providers, other healthcare professionals, team members, clinic staff, internal or external services, and community agencies. The RPM-HTCC RN monitors and tracks program statistics and initiates and assists in performance improvement activities for the program as directed by the Nurse Manager. The RPM-HTCC RN serves as a member of the health care team and utilizes positive and effective interpersonal and collaborative skills. The nurse established professional relationships with Veterans, peers, families, and health care providers.
