Remote Telehealth Licensed Practical Nurse- FL License Required

Salary: $65,000-$70,000 (Negotiable, depending on experience) plus health benefits, PTO, 401k with match, and more Job Summary: Our client is seeking a Licensed Practical Nurse (LPN) to support patients with chronic neurological conditions through remote Principal Care Management (PCM) and Chronic Care Management (CCM) programs. This role focuses on proactive care coordination, patient engagement, and clinical support between provider visits. Job Responsibilities: • Conduct monthly and ongoing patient check-ins via phone or telehealth • Support care management for patients with neurological conditions • Coordinate referrals, appointments, and medication reminders • Document care plans and time-based activities for Medicare compliance • Communicate with caregivers, clinics, and supervising RNs • Identify and escalate changes in patient condition • Assist with scheduling, documentation, and EMR updates Qualifications: • Active, unrestricted LPN license in Florida (multi-state license a plus). • Strong communication, empathy, and interpersonal skills when engaging with patients and caregivers. • Proficiency with electronic medical records (EMRs) and digital health platforms. • Highly organized, reliable, and proactive in managing multiple patient caseloads. • Experience with Medicare PCM/CCM or similar chronic care programs. • Background in caring for older adults or neurodegenerative disease populations. • Familiarity with telehealth or hybrid care delivery (preferred). • Exposure to quality improvement or care coordination initiatives.
