About the position As a Private Duty LVN, you will deliver high-quality, one-on-one skilled nursing care to seniors and adults with disabilities in the comfort of their homes. Responsibilities • Provide exceptional hands-on care, including wound care, medication management, and the ongoing monitoring of chronic conditions. • Collaborate closely with physicians, rehabilitation services, and family members to ensure seamless, comprehensive care. • Empower clients and their loved ones by educating them on diagnoses, treatment plans, and wellness strategies. • Maintain highly organized, accurate, and timely clinical documentation of the exceptional care you provide.
