Position Summary: We are seeking an experienced and empathetic Licensed Practical Nurse (LPN) to join our team as a Remote Care Manager. In this role, you will engage with patients virtually to assess, coordinate, and monitor their health care needs—playing a vital part in our mission to enhance quality of life through proactive care management.
