California Licensed Vocational Nurse

A company is looking for a Licensed Vocational Nurse (CA license required). Key Responsibilities • Support delivery of health care by performing activities prescribed by Nurse Practitioners and clinical policies • Provide one-on-one coaching to patients managing chronic conditions • Conduct comprehensive assessments and monitor patients' progress and adherence to treatment plans Required Qualifications • Active multi-state compact Practical Nurse License • Active California State LVN license required • 5+ years of clinical experience as a Licensed Practical Nurse • Experience in patient education for individuals managing chronic conditions • Prior experience supporting patients remotely is a plus
