California Licensed Telehealth Nurse
Job Description
A company is looking for a Telehealth Heart Failure Nurse.
Key Responsibilities
• Monitor and screen incoming alerts, calls, and chat communications
• Conduct patient onboarding and schedule audio-visual calls with patients
• Communicate patient concerns and report abnormal values to the Provider Team
Required Qualifications
• Current, active LVN license in California in good standing
• Minimum of 2 years hands-on nursing experience; telehealth experience preferred
• Fluency in English and Spanish
• Experience with patient phone triage in-office or via telemedicine
• Ability to work from home in a HIPAA compliant environment
