Licensed Vocational Nurse (LVN) – PRN Chronic Care Management (CCM) & Remote Patient Monitoring (RPM) Remote | Texas or Florida REQUIRED About Us Advanced Clinic Testing partners with primary care providers to deliver structured, compliant, and performance-driven CCM and RPM services. Our programs are built to improve patient outcomes, strengthen quality metrics, and maintain strict alignment with CMS and MIPS standards. Position Overview We are seeking an experienced LVN (PRN) with a strong background in CCM and/or RPM. This role is ideal for a nurse who is comfortable working independently, understands Medicare compliance requirements, and can step in as needed to support patient follow-ups, documentation oversight, and workflow optimization. This is a PRN, remote position offering flexibility with clear performance expectations. Key Responsibilities • Conduct monthly CCM and RPM patient follow-ups • Ensure documentation meets CMS time and compliance requirements • Accurately track time and support proper code eligibility • Review care plans and chronic condition documentation • Communicate updates to providers and internal care teams • Monitor productivity metrics (completed touches, engagement rates, code capture) • Assist in maintaining structured workflows and standardized documentation • Provide coverage or support for other team members when needed Qualifications • Active LVN license (Texas preferred) • Minimum 2+ years CCM and/or RPM experience • Strong understanding of Medicare CCM/RPM billing guidelines • Experience with EHR systems • Highly organized and detail-oriented • Comfortable working independently in a remote environment • Strong professional communication skills Compensation & Structure • $22–$27 per hour (PRN), based on experience • Performance expectations tied to monthly productivity and documentation quality • Flexible weekday scheduling • Fully remote position High-performing candidates may have opportunities for expanded responsibilities and performance-based incentives. Qualifications
