Role Overview
Alignment Health is looking for a Remote Nurse Practitioner / Physician Assistant for the Virtual Care Center (VCC) to deliver telehealth services to senior patients.
Responsibilities
- Conduct virtual visits and comprehensive assessments (past medical history, symptom review, physical exams, medication review, and cognitive/depression screenings).
- Formulate diagnoses, document findings, and generate post-visit summaries for Primary Care Providers (PCPs).
- Educate members on chronic conditions, self-care, sick-day plans, medication compliance, and community resources.
- Participate in weekly interdisciplinary team meetings with RNs, social workers, health coaches, and coordinators.
- Ensure compliance with HIPAA regulations.
Requirements
- Active Nurse Practitioner with RN licensure or Physician Assistant board licensure.
- Master’s degree from an accredited NP or PA program.
- At least 1 year of prior clinical experience.
- Valid BLS certification upon start; DEA number and NCCPA board certification within 6 months of start.
- NPI number upon start.
- Openness to a flexible schedule supporting a 24/7 operation (day, evening, overnight, weekend, and holiday shifts).
