Licensed Vocational Nurse LVN (Home Health) in the city of (Banning, CA) and (Corona, CA)

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. Duties • Provide direct patient care in the home setting under the supervision of a Registered Nurse. • Assist with medical procedures such as catheterization, wound care, and tube feeding. • Initiates and identifies client's needs through assessment • Utilize electronic health record (EHR) data soft logic to document patient care and ensure HIPAA compliance. • Provide accurate and timely documentation consistent with the plan of care, prepare coordinate, and submit clinical progress notes. • In collaboration with the treatment team, participates in the organization and management of medication coaching and delivery to clients, as well as integrating medication administration into treatment plans. • Collaborate with patients primary care physician to coordinate patient care plans, and discharge planning as needed. • Educate patients and families about care instructions, treatment plans, and health maintenance. • Respond to patient needs promptly with professionalism and compassion. Requirements • Current CA LVN License (Required) • 2 years of LVN experience in Home Health • Graduation from an accredited LVN program • Current CPR/BLS certification • Ability to chart with Electronic Medical Record Software Pay: $40.00 - $45.00 per hour Work Location: Remote
