Job Description
Most telehealth roles ask you to trade clinical depth for convenience. This one doesn't. A fast-growing value-based care organization serving older adults in underserved communities near Bakersfield, California is hiring an FNP for a primarily remote position that combines the flexibility of working from home with genuinely complex, meaningful clinical work — managing CKD, ESKD, and chronic conditions in a senior Medicare population that benefits enormously from consistent, longitudinal care.
Monday–Friday, no nights, no weekends, in-clinic just 1–2 times per month. The rest is yours to manage remotely on a flexible schedule built around quality outcomes, not patient volume. Key Responsibilities
• Manage medically complex senior Medicare patients with CKD, ESKD, and chronic conditions
• Develop individualized care plans focused on quality outcomes over patient volume
• Perform Comprehensive Health Assessments and Health Risk Assessments
• Coordinate care with physicians, families, dialysis centers, and integrated care teams
• Provide patient education and clinical guidance via telehealth and monthly in-person visits
• Manage medications and conduct post-discharge follow-up
• Administer tests, screenings, and accurate clinical coding and documentation
• Support patients navigating complex healthcare systems
Qualifications
• Active, unrestricted California FNP license
• Board Certified NP (ANCC or AANP required)
• Minimum 2 years NP experience in primary care, family medicine, internal medicine, nephrology, gerontology, or palliative care
• DEA licensure and prescriptive authority required
• Active CPR/BLS certification required
• Active California driver's license required
Schedule & Compensation
• $150,000–$160,000/year + $10,000 sign-on bonus
• Full-time | Monday–Friday | Flexible schedule
