Job Description

Telehealth After Hours Nurse Practitioner A Telehealth After Hours Nurse Practitioner position entails providing immediate medical assessment and treatment to residents outside of regular business hours, responding to urgent calls from nursing staff regarding patient concerns, and making necessary clinical decisions while collaborating with the primary care team, all virtually while the patient is within a nursing home. Responsibilities: • Responding to after-hours calls: Answering phone calls from nursing staff regarding patient concerns like changes in vital signs, sudden symptoms, or medication issues. • Conducting medical assessments over the phone or video: Evaluating patient conditions based on reported symptoms, reviewing medical records, and performing necessary physical examinations when required. • Making clinical decisions: Diagnosing acute medical issues, ordering appropriate diagnostic tests, prescribing medications, and initiating necessary interventions based on patient needs. • Collaborating with nursing staff: Providing guidance and support to nursing staff on managing urgent patient situations, including medication administration and symptom management. • Communicating with primary care providers: Updating primary care physicians on urgent patient issues, coordinating care plans, and seeking consultation when necessary. • Documenting patient encounters: Thoroughly documenting all assessments, interventions, and communications in the electronic medical record. Qualifications: • Valid Nurse Practitioner license: Current and active license to practice as a Nurse Practitioner in the state of employment. • Experience in geriatrics: Prior experience working with geriatric patients in a nursing home setting is highly preferred. • Strong clinical judgment: Ability to quickly assess patient conditions and make appropriate clinical decisions in urgent situations. • Excellent communication skills: Clear and effective communication with nursing staff, patients, and family members. • Availability for on-call shifts: Commitment to being available outside of regular business hours, including nights, weekends, and holidays.
